Weapon - Kaom's Primacy. This is pretty much the best axe I've seen for this build. It has high damage, and Culling Strike is just amazing.Helmet - Rat's Nest. Increases attack speed, gives you some nice crit, and it adds to your movement speed and rarity, both of which are huge quality of life improvements.
For everything else, focus on resists and life as a priority. Make sure you have at least 20% increased movement speed on your boots, too.
Guide
This is a fairly straightforward build. Using the gems listed above, your Spectral Throw will be able to hit pretty much everything on the screen while leeching massive amounts of life back to your character. You can spam Spectral Throw thanks to Blood Magic, and the damage over time caused by burn damage is extremely useful for tedious fights such as the Devourer boss in the new Orchard map. For those fights (Devourers, Brutus, etc), you'll want to quickly cast one Spectral Throw then strafe a bit before casting again to ensure that you don't get hit with their devastating attacks. This is why movement speed is so important.With the new Cast When Damage Taken gem, your character will spam the attached skills automatically. This happens often enough that your curse is always on the enemy without you needing to waste a second to cast it manually. Molten Shell provides some nice survivability and mediocre damage. If you have enough links available, Rejuvenation Totem provides a small distraction to enemies, and bolsters your overall survivability. Ice Nova slows enemies a bit, but it might become difficult for you to accumulate enough intelligence to level it up very high. Do not level up the Cast When Damage Taken gem - it will increase the damage of the attached skills (not that important - this is for survivability anyway) while making it more difficult for the attached skills to proc.
If you get into a pinch, cast Immortal Call and either Leap Slam out of there, or reposition yourself while your Life Leech and flasks heal you back to full. You have more than enough emergency buttons to press in case you get into trouble, even if you're up against a physical reflect pack. Between your flasks, life leech, and Cast When Damage Taken skill spam, there's not much that you will struggle with.
As you become more accustomed to this build, you can try casting a bunch of Spectral Throws at a pack of mobs, then immediately leap slam backward so that your thrown weapon has a longer travel time and will hit the enemies you just jumped behind as well.
For your flasks, one life flask should be a long heal over time, while the other should be one of the new sanguine flasks or an instant heal flask. Your Granite Flask can be swapped out with a Jade Flask since we have Iron Reflexes anyway. And if you happen to be struggling with Chaos damage mobs, feel free to swap one of the Quicksilver Flasks out with an Amethyst Flask.
As for which passive skills to take first: use common sense. If you find that your survivability is lacking, invest in more life nodes. If you find that you aren't killing mobs quickly enough, invest in more damage nodes. You can also branch off of the main path and invest in some resistance nodes if you need to. You can always refund them later once your gear catches up.

Why do you go for master of the arena? I thought that melee damage didnt increase spectral throw.

lolXDsorandom is right, any skill with "melee" in the description does not effect Spectral Throw. (minus Iron Grip, of course) However, I do like your use of "Cast When Damage Taken". I didn't know that it removes mana and animation. Gonna have to play with that aspect myself. :)

Master of arena is a stepping stone to 2hand skills and life nodes, the added strength is the bonus, so you can go this route which nets you 20%life 8% mana +2 melee range +20 strength or go thru the bottom row and take +10 dex +10 strength and 4 skill points to spend elsewhere. personally I would take those four points and put them in the two handed nodes under unwavering stance, here those four points would give you 15% stun duration 37% physical dam 10% life 20% accuracy
